About Dr Haseena Mohamed
Dr Haseena Mohamed (MBChB, FRACP, IBCLC) is a General Paediatrician
Consultant and board-certified Lactation Consultant with over 15 years'
experience providing care for families across Brisbane.
She is committed to providing high quality care to her patients and
cherishes providing comprehensive and thorough paediatric care. She
gives full attention to treatment needs and expectations of the children
as well as the hopes and concerns of their parents - all in a welcoming,
kind, friendly and supportive atmosphere.
Dr Haseena Mohamed cares for children from birth to 17 years of age.
She has a special interest in newborn babies and supporting parents with
breastfeeding difficulties. She also cares for children with
developmental and behavioural difficulties.
Availability and Consulting Hours
Dr Haseena consults from Suite 4, 16 Weippin Street, Cleveland, on
Fridays and most Wednesdays usually from 9am to 4pm.
Appointments and other enquiries can be scheduled by phone (NOT via
email or SMS) between Monday to Friday from 10am to 4pm on 0499551417.
In case of a missed call, we will endeavour to get back to you as soon
as we can depending on the ability of our staff to respond as they
may be attending to other duties.
For medical emergencies, please call 000, or present to nearest
emergency department of a hospital. We are not emergency service
providers.
Please allow about 2-3 weeks after fee pre-payment is completed for
Dr Haseena to reply to requests via correspondence for scripts letters
and forms to be filled out and emailed back to the patient.
Please call receptionist to book a phone consultation appointment for
brief enquiries related to the treatment. These cannot be answered via
email.
Pre-payment will be required at the time of request prior to these
services.
Accounts and Billing
Gentle Care Paediatrics is not a bulk billing practice.
Full payment of fees is required on the day of consultation at the
appointment.
Gentle Care Paediatrics reserves the right to require the fee to be
paid upfront before booking the appointment or any time before the
appointment.
All fees are required to be paid in full before booking the next
appointment. Failure to pay automatically cancels all subsequent
appointments even the future ones that were confirmed before the failure
to pay the fee of an appointment.
Gentle Care Paediatrics reserves the right to refuse treatment to a
person and their financial dependants in cases such as, and not limited
to, failure to pay fee, follow administrative procedures, or respond to
our messages repeated cancellations, not 'showing-up' for a confirmed
appointment, or uncooperative or difficult conduct towards any of the
staff or fellow patients.
Medicare Rebates
With a valid referral, patients may be eligible for a Medicare
rebate. Please email to us the referral before an appointment can be
made.
The amount of Medicare rebate you will receive back is determined by
Medicare and not by Gentle Care Paediatrics.
Gentle Care Paediatrics is not responsible for how much or any
Medicare Rebate that you receive.
Gentle Care Paediatrics only provides you with an
indicative/estimated Medicare Rebate amount, but it is up to Medicare to
determine what rebate you may receive depending on Medicare's own
criteria and your circumstances.
Gentle Care Paediatrics can send your rebate claims directly to
Medicare within a week after your appointment provided you gave us days
prior to the appointment a valid referral form, your current and correct
Medicare details and have registered your bank account and other
information with Medicare.
Usually, you can expect to get the rebate back from Medicare in a
week to 7 business days after the appointment.
If you do not receive the rebate by then, please contact Medicare
directly with the invoice and receipt of payment that you received from
Gentle Care Paediatrics at the end of your appointment.

Initial Appointments
An initial appointment for your child can only be made once a valid
referral is received.
For you to get the most Medicare Rebate for your appointment with the
Paediatrician, ensure you get a referral from a GP. Referral from
another Specialist Doctor or indefinite referrals usually have lower
Medicare Rebates.
We require that you have a referral letter addressed to the
Paediatrician at Gentle Care Paediatrics, Dr Haseena Mohamed. We do not
accept referrals that are addressed to someone else to provide the
treatment.
If you have such a valid referral letter, please email it to: Reception@GentleCarePaediatrics.com.au
in a clearly readable pdf format.
Once the referral has been emailed to us, after that please call and
speak to our reception staff on 0499551417 to book the appointment.
Be prepared to stay on the phone for about 10 minutes to give all the
required Medicare related information of the child patient and the
parent guardian to book the appointment. Following that, the
receptionist will give you appointment times that are available for you
to choose one that suits you.
You will receive a confirmation message via email with details of the
appointment that has been booked. Please note that until you receive
this confirmation email from us clearly confirming the appointment, the
appointment is not considered to have been confirmed.
When you receive a reminder email or SMS text message (usually a week
before the appointment) that asks you to reply to confirm whether you
are attending, you are required to reply to confirm that you are
attending. Please do not ignore this message.
When you arrive at our Specialist Suites waiting room for the initial
consultation, you need to complete a Patient Information Form and sign a
consent form for the privacy, confidentiality and to authorise Gentle
Care Paediatrics to provide treatment of your child. Please arrive a few
minutes early for this essential consent.
Please come prepared to wait with your child in the waiting room in
case of delays. Leaving at this stage is considered a 'no-show' at the
appointment and incurs a no-show/cancellation fee.
Please bring the current referral letter and any information (such as
health/medical reports and test results) pertaining to your child's
medical condition and past medical conditions, including a medication
list.
It is important you also bring to the appointment a copy of any
allied health assessments performed including blood tests and
X-rays.
Some of these essential documents (e.g. referral letter or test
results) occasionally may not be returned to you so please ensure you
keep a copy if you require it.
Letters or documents that you email to reception, especially only a
day or two prior to the appointment, may not be available to the doctor
at the appointment.
It is always best to bring a paper copy of any document that you want
to show the doctor at the appointment. Please note we strictly do not
accept USB drives etc to be inserted into our any of our electronic
systems at the Clinic.
Your initial appointment will usually be of up to one hour duration.
During this time the Paediatrician will take a thorough history and
examine your child.
A follow-up appointment may be arranged at the end of the
appointment, or alternatively please call reception on 0499551417 later
to schedule a follow-up review appointment if required.
It is your responsibility to call and request any appointments
whether it is an Initial Consultation, Review/Follow-up or Phone
Consultation appointment. Appointments cannot be made in any other way
such as via emails, SMS or voice messages.
Remember to keep any pharmaceutical prescription scripts and letters
issued by Gentle Care Paediatrics (doctor or staff) as should these need
to be re-issued after the appointment, you will likely incur an
additional fee. Usually, you can get repeats from your GP instead and
will not need to see the Paediatrician for repeats of Scripts. Please
ask the Paediatrician at the appointment for Scripts, letters or forms
to be completed during the appointment to avoid fees if requested at a
later time.
Follow-up Review appointments
Follow-up review appointments are usually scheduled for 30
minutes.
Sometimes these will be booked by the Doctor at the appointment
itself.
If a review appointment was not booked during the appointment by the
Doctor, then it is the patient's parent/guardian's responsibility to
call the receptionist and ensure a follow-up review appointment is
completed with the reception over the phone on 0499551417 (and not via
emails or messages).
Please do not expect the receptionist to chase the parent to make any
appointment.
Once the review/follow-up or Phone Consult appointment is booked, the
receptionist will send an email to confirm it with its details.
New-Baby Check appointments
Please follow the above procedure as with Initial Consultation
appointment booking.
Once your baby is born, it is essential that you call the reception
on 0499551417 or email the reception on Reception@GentleCarePaediatrics.com.au
well before the appointment to give the baby's date of birth and name to
the receptionist.
Likewise, please inform the receptionist of the baby's and the
parent's updated Medicare Card details when you know them.
Appointments - General
Dr Haseena will send a report/letter back to your referring doctor
after each consultation.
Please note that Gentle Care Paediatrics staff are here to provide a
service to you as best as they can, so please comply with their requests
and instructions related to management of your child's condition.
Gentle Care Paediatrics reserves the right to cease providing our
services to the patient and the family of the offending parent/guardian
in case of non-compliance or unsatisfactory conduct.
Whenever the child or the parent/guardian's Medicare Card, home
address or contact phone and email change, it is essential and very
important that you inform us of the updated details via email or phone
call immediately. Failure to do this is likely to seriously affect
Medicare Rebates that you receive from Medicare, appointment
confirmations from us and more.
Parking
Parking is available at the front of the Mater Health Centre Redland
Building in the Specialist Suites at Bayside Business Park and in nearby
areas.
Form Completion and Letter Writing
Should you require NDIS Access forms, Centrelink forms, or any other
forms or letters required by school or Child Safety offices, please call
Gentle Care Paediatrics.
Gentle Care Paediatrics will charge a pre-payment fee for this
service which is payable upfront. These fees range $50-$100 depending on
the requirements.
Completion of required forms can take up to 2-3 weeks. We cannot
provide these at short notice.
There is no Medicare rebate available for this service.
To avoid these fees afterwards, please request, and whenever
possible, take any letter you need from the Doctor during the
appointment itself.
Scripts/Prescription
Your GP can later provide you with repeat scripts for PBS Authority
medications once they have been initiated by the Gentle Care
Paediatrics' Paediatrician at your appointment.
If you require Gentle Care Paediatrics to provide you with repeat
scripts after your appointment, Gentle Care Paediatrics charges a
pre-payment fee of $30.
Scripts can be emailed to you and take up to 1-2 weeks after the
pre-payment.
If the matter is urgent Gentle Care Paediatrics recommends you see
your GP.
Please note that there is no Medicare rebate available for the fee
paid for writing a pharmaceutical script.
Please note that Gentle Care Paediatrics treats many other
patients who also have very strong needs and therefore Gentle Care
Paediatrics cannot guarantee the completion of out-of-the-clinic
requests for form completion, letter writing and scripts at short notice
or in short time. Typically, it may take between 2-3 weeks.
Appointment Cancellation & No-Show
Policy
We understand that issues may arise where an appointment may need to
be cancelled or rescheduled.
Please notify us as soon as possible if you need to cancel or
reschedule an appointment as other patients waiting can be seen
earlier.
Notification to cancel or reschedule should be via an email, sms or
speaking to our receptionist on a phone call (but not by voice
messages).
Gentle Care Paediatrics requires a notification at least 48 hours fom
business days prior to the start of the appointment to avoid you being
charged a cancellation fee of $100. Request to
reschedule or cancel within 48 hours also incurs a fee of $100.
Patients who do not attend their appointment ('no-show') without
48-hours' notice prior to the appointment will be charged a cancellation
fee of $100.
In the case of an unexpected late illness within the 48 hours prior
to the appointment, or even before that, please notify us and
alternative arrangements for a phone review consultation may be possible
in some cases when needed. An initial appointment needs to be in person
and cannot be a phone consultation.
Whilst every endeavour is made to run on time, Gentle Care
Paediatrics may from time to time not be running to schedule due to
circumstances beyond its control. Please ensure you come prepared for
unexpected delay in the waiting room due to a previous appointment and
wait for your appointment.
Should you leave the waiting room prior to your appointment you will
be considered as cancelling your appointment and the cancellation
('no-show') fee of $100 will be payable.
An appointment cannot be made until all outstanding fees are paid.
Even previously confirmed appointment bookings for future appointments
are automatically deemed to be cancelled until all fees are paid.
Gentle Care Paediatrics reserves the right to take full fee
pre-payment before booking an appointment. In such cases, those full
pre-paid fees are forfeited for subsequently not attending the
appointment.
Gentle Care Paediatrics reserves the right to cease providing the
services at any time to any patient, including the financial dependants
of parent/guardian's who did not pay a fee, were uncooperative with the
staff, unreachable despite attempts to contact regarding the
treatment/services being provided, or were of unsatisfactory conduct
towards the staff or other patients.
Please see the next section about how to pay these fees in a timely
manner.
Please note that Gentle Care Paediatrics strives to provide the
best treatment and service to the patients and your collaboration with
our staff in the processes, including cancellation or rescheduling of
appointments, helps us to cater for all patients, including you, in the
best and most timely manner.
How to Make Appointment, Fee Payments, Pre-payments, Payments
for Scripts, Letters and Forms?
A next appointment can only be scheduled, deemed confirmed or
conducted after paying all prior appointment fees including cancellation
fees.
Appointment Fees:
Please pay appointment fees after the appointment at the Specialist
Suites. The invoice you receive after your payment at the appointment,
please keep it with you at least for a few weeks in case if you need it
for Medicare Rebate claims.
Pre-payment of Fees:
In some cases, Gentle Care Paediatrics reserves the right to take a
prepayment even before booking an appointment or days before an
appointment.
Routine pre-payments are required before booking a Phone Consultation
appointment, accepting to write a pharmaceutical script, a letter or a
form. Sometime pre-payment maybe taken for a routine appointment.
For pre-payments and payments that are required outside of the
Specialist Suites, the Receptionist issues you an invoice via Square
payment systems for the payment. Then you will receive an invoice from
Square payment systems in your email address that you provided us as
your official contact with us. The email contains a link for you to
click on and make the payment yourself. If you like you can confirm that
this is from us by calling or emailing our receptionist.
Please do not send any credit card information to us over an email,
SMS or any other form of a message. Gentle Care Paediatrics will never
ask you to give us your credit card details over an email, website link,
SMS, or any other message and we do not store your credit card details
with us.
Privacy Policy and Confidentiality
Gentle Care Paediatrics respects your privacy, and your personal
information is held in the strictest confidence and with the highest
priority. We only take your information that is necessary for the
treatment, and we store your information with strict privacy and
confidentiality.
Gentle Care Paediatrics only collects from you information that is
needed for your treatment, Medicare rebates, accounts and correspondence
with you.
The information we collect from you for this includes:
Contact phone number and email address of the
parent/guardian
The patient's full name, dob, address, Medicare number with
expiry date and the same information about a parent/guardian who will be
the account holder for the child patient.
Name and clinical contact of your referring doctor, usual GP and
sometimes other specialists involved in your treatment.
The Doctor takes from you your medical history and such
information related to your presenting condition that is being
treated.
As such, Dr Haseena Mohamed and Gentle Care Paediatrics Pty Ltd Staff
may need to request records from, or share information needed for your
treatment with, your GP, current or previous Specialist Doctors or other
health providers to assist with your treatment planning. We also may
correspond and forward x-rays and test results/reports when required
with your General Practitioner or other specialists for treatment
planning. During your treatment, we may need to refer you to other
specialists & health providers. To ensure compliance with Federal
and State Privacy Legislation we require your signed consent prior to
your initial appointment to work with other healthcare
professionals.
If any of the information Gentle Care Paediatrics has about you is
incorrect or changed (such as an updated Medicare Card number), please
inform our reception staff via phone call.

1. Introduction
Gentle Care Paediatrics is committed to best practice in relation to
the management of information we collect. This practice has developed a
policy to protect patient privacy in compliance with the Privacy Act
1988 (Cth) ('the Privacy Act'). Our policy is to inform you of:
the kinds of information that we collect and hold, which, as a
medical practice, is likely to be 'health information' for the purposes
of the Privacy Act.
how we collect and hold personal information.
the purposes for which we collect, hold, use and disclose
personal information.
how you may access your personal information and seek the
correction of that information.
how you may complain about a breach of the Australian Privacy
Principles and how we will deal with such a complaint.

2. What kinds of personal information do we
collect?
The type of information we may collect, if needed, and hold
includes:
Your name, address, date of birth, email and contact
details
Medicare number, DVA number and other government identifiers,
although we will not use these for the purposes of identifying you in
our practice
Other health information about you, including:
notes of your symptoms or diagnosis and the treatment given to
you
your specialist reports and test results
your appointment and billing details
your prescriptions and other pharmaceutical purchases
your treatment related genetic information if required
your healthcare identifier
any other information about your race, sexuality or religion,
when collected by a health service provider - we usually do not collect
this, unless needed for treatment.
3. How do we collect and hold personal
information?
We will generally collect personal information:
from you directly when you provide your details to us. This might
be via a face-to-face discussion, telephone conversation, registration
form or online form
from a person responsible for you, such as your parent
from third parties where the Privacy Act or other law allows it -
this may include, but is not limited to: other members of your treating
team, diagnostic centres, specialists, hospitals, the My Health Record
system, electronic prescription services,
Medicare, your health insurer, the Pharmaceutical Benefits
Scheme
4. Why do we collect, hold, use and disclose personal
information?
In general, we collect, hold, use and disclose your personal
information for the following purposes:
to provide health services to you
to communicate with you in relation to the health service being
provided to you
to comply with our legal obligations, including, but not limited
to, mandatory notification of communicable diseases or mandatory
reporting under applicable child protection legislation.
to help us manage our accounts and administrative services,
including billing, arrangements with health funds, pursuing unpaid
accounts, management of our ITC systems
for consultations with other doctors and allied health
professional involved in your healthcare;
to obtain, analyse and discuss test results from diagnostic and
pathology laboratories
for identification and insurance claiming
If you have a My Health Record, to upload your personal
information to, and download your personal information from, the My
Health Record system.
Information can also be disclosed through an electronic transfer
of prescriptions service-Escript.
To liaise with your health fund, government and regulatory bodies
such as Medicare, the Department of Veteran's Affairs and the Office of
the Australian Information Commissioner (OAIC) (if you make a privacy
complaint to the OAIC), as necessary.
5. How can you access and correct your personal
information?
You have a right to seek access to, and correction of the personal
information which we hold about you
For details on how to access and correct your health record, please
contact our practice as noted below under 'Contact Details'.
We will normally respond to your request within 30 days.
6. How do we hold your personal information?
Our staff are trained and required to respect and protect your
privacy. We take reasonable steps to protect information held from
misuse and loss and from unauthorised access, modification or
disclosure. This includes:
Holding your information on an encrypted database with strong
password protection
Holding your information in secure cloud storage
Our staff sign confidentiality agreements
Our practice has document retention and destruction
policies
7. Privacy related questions and complaints
If you have any questions about privacy-related issues or wish to
complain about a breach of the Australian Privacy Principles or the
handling of your personal information by us, you may lodge your
complaint in writing to (see below for details). We will normally
respond to your request within 30 days.

8. Anonymity and pseudonyms
The Privacy Act provides that individuals must have the option of not
identifying themselves, or of using a pseudonym, when dealing with our
practice, except in certain circumstances, such as where it is
impracticable for us to deal with you if you have not identified
yourself.
9. Updates to this Policy
This Policy will be reviewed from time to time to take account of new
laws and technology, changes to our operations and other necessary
developments. Updates will be publicised on the practice's website.
10. Privacy and websites
This policy is available on the practice website. Your personal
information will be collected over the phone, email and on the paper
patient consent form.
